I would assume they're talking about full-rate, fully-pipelined oprations. So no single operation spends 12000 pJ per clock, but a steady stream of them do.

For example, 3 channels of DDR3-1066 memory will give you 3 billion 64-bit reads per cycle. (Assuming a good hit rate on the open rows.)
